Skills For Care – Help Investigating Medicines Errors

Skills for Care has launched a new resource to support safer medicine practice. The ‘Contributory factors framework for medicines errors in adult social care settings’ helps services investigate errors in a structured, learning‑focused way. As the CQC assesses medicines optimisation under Safe care, taking prompt, informed action on errors is essential for meeting expectations and improving safety.

The interactive framework highlights key factors to consider and supports a root‑cause‑analysis approach, encouraging staff involvement and a culture of openness. It includes case studies and a summary and action‑planning template to help prevent future errors and strengthen safe practice.

An excellent doctorate research article by one of our board members – Martha Pusey – about how storytelling can shape future care for people living with dementia, so they can express what matters most to them before moving into residential care.
